If your child has contracted an infectious disease such as pharyngoconjunctival fever (adenovirus), measles, rubella, chickenpox, mumps, mumps, or enterohemorrhagic E. coli infection (O157), and has recovered and returned to school. Please submit this document when reopening the nursery school. Please print this out and have your doctor fill it out.

If your child has streptococcal infection, respiratory syncytial virus infection, hand, foot and mouth disease, viral gastroenteritis, sudden rash, herpangina, erythema contagious, herpes zoster, or mycoplasma pneumonia, please submit this notification to attend school. This will be filled out by the parent/guardian. Your child may resume attending school once he/she has recovered to the level specified in the "Guidelines for Attendance" written in this paper.

Please submit this Influenza Treatment Report when your child becomes ill with influenza. This is to be filled out by the parent/guardian. Please confirm the onset date with your doctor at the time of your visit.
